Endovasal laser coagulation (EVLK) of the veins of the lower extremities in Moscow
Endovasal laser coagulation (EVLK) of veins in Moscow
Endovasal laser coagulation (EVLK) of veins is a modern method of getting rid of stem expansion of veins using thermal exposure to laser energy. At the same time, the vein is preserved, just as with a standard, familiar operation, it is burned out from the inside. This contributes to thrombotic occlusion of the vein, its compression and gradual resorption. The technique is carried out under the control of a special scan - color duplex, which allows not to cut the skin, but to carry out the procedure through tiny punctures.

EVLK - endovasal laser coagulation in our center
The first joyful reports about the possibility of using a laser in the treatment of phlebology patients appeared in 1981. Dr. Anderson RR and colleagues used a dye laser having a wavelength of about 577 nm, the principle was to damage the microvessels of the skin. In the development of the foundations of technology, the process of absorbing the energy of a laser with a certain wavelength was laid by the tissues, and this resulted in their selective destruction.
Only 90-ies of the last century, when new semiconductor structures saw the light, it became possible to produce compact lasers. These devices had a great service life at a fairly low cost. In 1998-1999, the first information about the beginning of clinical manipulations based on intravascular application of a diode laser for endovasal laser coagulation began to appear. As a result, only in 2002 this method was patented by V.Meloni and his colleagues.
Since the advent of the method - endovasal laser coagulation of varicose veins, the video of which was filmed for a more in-depth study of the process, a tendency has been observed to increase the power of energy that is supplied to the vessel. The earliest work was carried out at a power of not more than 15 W, and after research work carried out by a number of scientists, there were reports of large power indices not exceeding 40 W.
Endovasal laser procedure is performed by surgeons phlebologists Semenov A.Yu. and Kalachev I.I.
Endovasal laser coagulation of EVLK in Moscow is of great interest to both surgeons and their patients. Every year, the number of publications and studies on this topic is increasing. Endovasal laser coagulation of varicose veins, the price of which may vary depending on the equipment, is becoming more popular every year, thanks to patient education, is in great demand.
Despite all the existing experience, the technology of endovasal laser coagulation is still very far from perfect. Therefore, research continues in two directions: standardization of technology and refinement of indications, elucidation of the optimal wavelength of the laser beam.
The principle of performing the procedure of endovasal laser coagulation is based on the thermal effect of laser radiation on the inner part of the vein surface. But, as it was found out with the help of a huge number of different types of research, the laser does not fully affect the surface of the vessel walls. Most of the energy is absorbed by the blood contained in the vessel. In the blood, already under the influence of the light pulse, vapor bubbles are formed that contact the vein wall, as a result of which the thermal effect occurs. Further coagulation of proteins occurs in deeper layers due to damage to the endothelium.
It is the destruction of the endothelium that is of primary importance as a result of treatment. Part of the viable endotheliocytes, which in the future are an important source of wall regeneration, is retained. The destruction process should be complete, and this is possible only as a result of creating an optimal laser radiation energy density in the lumen of the vessel, which leads to its “carbonization”. However, you should carefully select the laser power, otherwise, with a stronger impact of the device, the vessel wall may perforate.
What happens to a vein during laser radiation during EVLK
The latest laser models for endovasal laser coagulation have a wavelength of 1940 nm, at which the laser radiation is absorbed to a greater extent precisely by the water of the venous wall and the blood itself. Directly acting on the vein wall reduces the radiation power, the appearance of carbonization and heating of the working surface. This contributes to a lesser probability of perforation of the vessel wall and the appearance of pain in the postoperative period. This wavelength is ideal for larger venous trunks that have a diameter of more than 10 mm.
Endovasal laser coagulation (EVLK veins), like any medical procedure, has both indications and contraindications.
Indications for the EVLK procedure:
- The estuary widening of the vein can be of absolutely any diameter. Since modern laser generators and radial optical fibers are able to close veins regardless of their diameter. All techniques for such manipulations are standardized and have excellent results.
- A fairly flat trunk of the vein. In the case of bends, it is difficult to pass through the endovasal conductor, in such cases several optical fibers are installed.
A difficult case - EVLK after a thrombophlebitis: several optical fibers were delivered
Contraindications to the EVLK procedure:
- Established thrombophilia, since the method of endovasal laser coagulation involves the formation of a thrombus near the lumen of the femoral or popliteal vein and the further clotting of the blood, which is a threat of the thrombotic process in the veins and thromboembolism of the arteries of the lungs.
- Chronic ischemia of the lower extremities, since an important stage of laser coagulation is the mandatory use of compression bandages for some time after surgery, which can aggravate ischemia.
- Lack of activation of the patient after surgery. It is known that the best way to prevent postoperative complications is to activate the patient. The method of endovasal laser coagulation requires that the patient get up from the operating table on his own and move for a certain time.
- The presence of inflammation in the area of intervention, which can serve as a source of infection.
Operation - endovasal laser coagulation (EVLK), video which you can look at on this page of our site, proceeds as follows: on a patient’s skin, marking is carried out using ultrasound control.
Kalachev I.I. carries out marking before the EVLK operation
Then, the lower border of reflux is directly determined, which is located at the point of accession of a large influx. The second mark is placed on 3-4 cm distally from the first, where puncture will be performed. Next, the inflow points of the tributaries are marked and varicose enlarged tributaries are marked, regardless of the method of removal.
Vein puncture is performed only under ultrasound control.
The technique of carrying out endovasal laser coagulation includes five steps:
- Puncture of the subcutaneous main vein and direct conducting of the light guide.
- Determination of the working area of the fiber.
- The beginning of anesthesia.
- Application of the laser.
- Band application.
After the compression hosiery is worn, the patient should walk for at least 40 minutes.
After the EVLK operation, compression knitwear is put on
24-hour compression lasts 2 days. Further only in the daytime for about a month. The first scheduled examination with ultrasound angioscanning occurs on the next day.
Rarely enough, more often in patients with a low pain threshold, complications are rare in the first 24 hours after the procedure - endovasal laser coagulation (EVLK). A slight soreness in the limb may be noted. Discomfort is more often associated with an improperly worn compression stocking, which compresses the limb, and not with postoperative injury.
Endovasal laser coagulation (EVLK), photos of the postoperative period of which demonstrate that bruising along the length of the coagulated vein is also possible, is the safest procedure for varicose veins. The cause of bruising can be getting into small capillaries during tumescent anesthesia. Sometimes there is a feeling of tension in the thigh, a feeling of tension in the vein. This all usually lasts for 1,5 months. Perhaps a short-term increase in body temperature to subfebrile figures.
About a fifth of patients, on 4-6 day after endovasal laser coagulation, a pulling sensation along the vein may appear - this is normal !.
Over the past 3 years, endovasal laser coagulation (EVLK), the cost of which is still quite high, has undergone several changes. For example, the most advanced laser devices have appeared, such as water lasers.
Modern laser technologies in the phlebology center "MIFTS"
Another important change is the formation of deep ideas of phlebologists about the presence of important indications for this technique. Endovasal laser coagulation of varicose veins, the reviews speak for themselves, have recently gained wide popularity and have gained trust among doctors and patients. Over the years, vast experience has been gained in using endovasal laser coagulation to eliminate venous pathology, and relevant new findings have been published in all world medical journals on phlebology.
The price for the endovasal laser coagulation of veins procedure in our clinic is strictly fixed, and therefore, turning to us, patients can count on high-quality service, treatment and affordable cost. And remember that a timely visit to a doctor will completely cure the disease and avoid its relapse.
Prices for innovative procedures of the phlebology center remain stable for many years. The cost of the endovasal laser coagulation procedure in one venous pool at the Moscow Phlebology Center is 59500 rubles. The price of the service includes:
- Local anesthetic benefit.
- The cost of a disposable laser light guide.
- The cost of consumables.

Well-coordinated work of the staff of the phlebology center "MIFTS"
Today, endovenous laser coagulation, the cost of which can be found by calling the medical center, is the safest type of surgical intervention for varicose veins in our country.
Today, Russia has accumulated vast experience in the application of endovascular laser operations on the veins of the lower extremities. Therefore, the risk of complications is minimized. They are so rare that isolated cases are discussed at conferences separately.
Endovenous laser coagulation, the photo of which can be viewed on the network, is quite painless, safe and easy to manipulate. Therefore, the demand for it is constantly growing. Every day, an increasing number of patients prefers its classic, outdated methods.

My grateful review about Dr. Phlebologist Fedorov Dmitry Anatolyevich.
I first met him in June 2010. At that time I was 57 years old. I have developed varicose veins on my legs since 30 years, and by this age I was already actively seeking the help of a phlebologist. A friend helped me, advised me to contact the Moscow clinic for phlebology problems, on Vasilisa Kozhina Street. The first examination was conducted by Dmitry Anatolyevich, said that he could offer an outpatient operation and soon performed complex surgical operations requiring accuracy.
This was followed by careful maintenance and correction during the 6 months, with strict implementation of recommendations. The treatment was not tolerated. The difficulties were different: the 2010 summer was very hot, there was smoke from forest fires in the cities, interruptions in the movement of electric trains, but all this was blocked by gratitude to the doctor for my “light” legs.
My disease is chronic. Time passed, I felt the need to see a doctor again and was about to call the Moscow clinic when I found out that a phlebologist was working at Kolomenskaya, in the MIFC clinic, and that was Dmitry Anatolyevich Fedorov. What a joy! THIS IS MY DOCTOR!
So in April 2017 of the year I met with Dmitry Anatolyevich for the second time, with his big skillful hands. This time he had me undergo laser surgery on both legs. The operation is tolerated very easily. After leaving the operating room, you can continue to engage in your normal life. And these are not words of advertising. This is the doctor’s possession of modern, high-tech treatment methods. I especially want to note that Dmitry Anatolyevich generously shares his professional knowledge, both of my operations were attended by doctors to whom he shared his experience.
Dear Dmitry Anatolyevich, I sincerely thank you for the health of my legs, for your high professionalism. Troubles Tatyana Nikolaevna, Obninsk. 04.10.2019
Testimonial of the patient about the EVLK procedure carried out in our center "MIFTS"
Maria, 41 year, Moscow region, Krasnogorsk.
Endovenous laser coagulation, the reviews of which I studied previously on the Internet, turned out to be completely painless. In the hospital, I spent 3 hours, which included the preoperative period, surgery, and 30 minutes after. I was pleased with everything. True, I had to go in special tights for a month, but it’s good that I took them off by the summer. There were only 20 small punctures in the lower part of the leg, now they are no longer visible. The leg does not hurt at all, otherwise the aching leg constantly ached before the operation. Thanks to the phlebologist, wonderful doctor Raskin Vladimir Vyacheslavovich for his good attitude and professional work. Mary, 41 year.
Patient feedback on endovenous laser coagulation surgery in our center
Stanislav, 47 years, Moscow.
The leg constantly hurt, there was a buzzing sensation in it at the end of the day. I couldn’t decide on the operation in the classical way, because it was scary after the reviews and possible complications read on the network. Before the operation, and I was offered - endovenous laser coagulation, in Moscow, at the MIFTs Phlebology Medical Center, where I decided to do it, did an ultrasound, donated blood and that’s all. After the operation, they were forced to get up from the table and walk for about an hour. The next day I saw a doctor, I was warned that compression stockings should be worn up to 3 weeks. I did not worry about scars, although I am glad that they are no longer visible. Six months have passed since the endovasal laser coagulation procedure. Everything is super. Thanks to all the staff of the Center for Phlebology "MIFTS" from doctors to nurses and especially to administrator Tatiana for their attention. Stanislav, 47 years.
