From September 30 to October 5, 2020 in the Conference Hall of the Tourist Hotel in Kaliningrad, within the framework of the Russian scientific symposium "THERMOPHYSICAL AND HYDRODYNAMIC PROCESSES IN MODERN PHLEBOLOGY AND LASER SURGERY", a Scientific and Practical Session on topical issues of modern phlebology was held.

The symposium was attended by both well-known representatives of fundamental sciences (Russian physics and mathematics) and specialists in the field of vascular pathology. Leading specialist of the Moscow Innovation Center of Phlebology, phlebologist, candidate of medical sciences, Dmitry Anatolyevich Fedorov (Moscow) made a report at the event.

Hovsep Petrosovich shared his experience of using modern endovascular technologies in his practice and world statistics on the treatment of varicose veins. After the introduction of endovascular techniques into widespread medical practice, we can safely say that the assistance to patients with venous pathology has risen to a different qualitative level. The treatment of varicose veins, in principle, has become the same outpatient procedure as going to the dentist. At the same time, the quality of medical care has only increased. With the help of modern techniques, we can finally completely control the problem of varicose veins in patients.

Roman Aleksandrovich presented a theoretical model of the formation of venous reflux and the development of varicose veins. Dr. Taugarinsky revealed the physical and biological principles of the formation of varicose veins. The primary link in the formation of varicose veins is a pathological change in the vein wall. How to influence this process, phlebological science has yet to answer this question.

The progressive growth of attention to aesthetic problems, as a result of the growing demand for cosmetic services, leads to the search for new solutions in aesthetic phlebology. Dmitry Nikolaevich presented the five-year practical experience of specialists from the phlebological center from Kaliningrad in the use of ozone-sclerotherapy for telangiectasia and reticular veins. The technique consists in introducing a gas mixture of ozone and oxygen into the venous vessels. In the course of clinical practice, the concentration of ozone in the gas mixture progressively increased. Today Dmitry Nikolaevich presented the method of using gas with 80% ozone content. Dr. Morenko shared the good results of the treatment, spoke about the nuances and practical aspects of the technology.
Konstantin Aleksandrovich Kaperiz, phlebologist, First phlebological center, member of NKF, member of S-POF, (Moscow), presented the message: "Laser 1940 nm, are there any advantages." Konstantin Aleksandrovich shared the results of a study of the physical parameters of the impact of the latest endovascular laser with a wavelength of 1940 nm. The latter demonstrated a number of significant differences between the 1940 nm laser from the previous generation of 1470 and 1570 nm. The fundamentally new physical parameters of the new laser open up the potential for endovascular interventions even more. A number of leading experts have already announced a change in the tactics of laser procedures when using a 1940 nm laser. The main advantage of the latter is greater safety and the ability to work with the suprafascial veins.

Professor Bogachev opened the topic of modern endovascular surgery in the context of the evolution of endovenous laser obliteration. Today, it is the laser treatment of varicose veins that is the leading and most rapidly developing technique in terms of technology. A certain disadvantage of the technology is the requirements for serious training of medical personnel, quality and reliability of equipment. Nevertheless, it is difficult to imagine a truly modern treatment of varicose veins without laser technologies.

Dmitry Anatolyevich gave a good historical excursion on the main milestones in the development of the laser obliteration technique, from the beginning of the 21st century to the present. Technology has changed significantly since its inception. The wavelength, power of laser radiation, laser light guides and delivery means of the latter were changed. Today, the laser obliteration technique allows a lot: to safely and effectively coagulate the varicose vein trunk along its entire length, to use minimal energy and even to abandon bulky introducer sheaths as a means of delivering a laser light guide. We can say that laser obliteration, as a technique, has reached its climax, and further development is impossible. What else can you think of? Not so long ago, the first reports appeared on the technique of total venous obliteration of varicose veins. Dmitry Anatolyevich was the first in Russia to use this endovascular technology in December last year. Today, the number of successfully performed procedures by Dr. Fedorov goes to dozens. Dmitry Anatolyevich supported the message with photographic documentation of good treatment results. The technical and methodological details of total laser obliteration were described in detail by the doctor during the report. Dmitry Anatolyevich also presented his vision of the prospects for the development of thermal endovascular techniques in the following:
- The emergence of thinner, but more resistant to thermal stress, radial fibers up to 0,7 mm, suitable for installation through conductors of the minimum diameter.
- Possibility of routine use of EVLK on small varices and even reticular veins; varicose veins of the feet, removal of unaesthetic veins on the forearms and hands of the upper extremities.
